Overview
The Form Field Download allows users with Form Access to download a ZIP file containing two CSV reports: a Form Field report with all form fields across all processes, and a Values report with any corresponding custom values. This file can be imported into Google Sheets, Apple Numbers, Microsoft Excel, or other spreadsheet applications for review, organization, or verification of form settings.
What the Form Field Download Includes
All form fields from all form processes.
Field rollover settings, showing whether each field is cleared during the yearly rollover process.
Custom values report, included in the downloaded ZIP file, which lists any custom values linked to your fields (matched via corresponding ID numbers in the form field report).

How to Access the Form Field Download
Navigate to: Settings > Forms
Click the Form Field Download button in the upper-right corner of the Forms page.
Your download will be provided as a ZIP file containing the CSV reports.
Form Field Report

Form Name:The name of the form where the field is located. -
Step Name:The step within the form where the field appears. -
Field Label:The question or title displayed to users on the front end of the form. -
Field Table:The data model or record where the field is stored in the system. -
Field Name:The unique system identifier for the field, used in reports, filters, and data exports. -
Field Type:The method used to collect data for the field (e.g., text, dropdown, checkbox). -
Is Conditional Logic?:Indicates whether the field is controlled by conditional logic within the step. -
Field Options ID:The ID assigned to the list of selectable options within a field (for example, Yes/No or Agree/Disagree). This ID can be used in the Form Field Options report to find the corresponding values for that field. -
Clear on Rollover:Indicates whether the field is configured to clear its value when the form rolls over to a new year or term.
Form Field Options Report

Field Options ID:The unique ID that links each option in this report to the corresponding field in the Form Fields report. -
Label:The text that appears on the form for users to see and select (for example, Yes or No). -
Value:The internal value stored in the system when a user selects that option. This is what appears in reports or exports, and sometimes may differ from the label.
